Job Description

Job Duties

·        Registered Nurse Case Manager Project Lead responsible for the management, business development growth, and overall performance of the contracts.  The RN Case Manager Project Lead is responsible for management of employees and timely completion of deliverables. The Lead is also responsible to assist the Director of Health in Expanding National Guard Case Management support contracts across the U.S.

·       The Case Manager Project Lead provides pre-mobilization IMR support to include identification and tracking of Soldiers who are not Fully Medically Ready (FMR} either for dental or medical concerns. Cases will be obtained from home station (HS} or mobilization station (MOBSTA) Soldier Readiness Processing (SRP) events for those Soldiers determined non-medically or non-dentally ready through other means (e.g. during their annual Periodic Health Assessment (PHA}.

·        Nurse Case Manager shall assist Soldiers to become FMR by assisting with · tracking medical and dental appointments, maintaining communications as required with the Soldier they are managing. All cases managed, regardless of type (medical or dental) or duration, must be documented in the eCase module of Medchart. Nurse Case Managers shall have a weekly team meeting with the Deputy State Surgeon (DSS), the State Surgeon {SS} and the State Dental Officer (SDO} to review all cases in progress.

·        The Nurse Case Manager shall be responsible to know the following ARNG webbased electronic data systems via Medical Operation Data System (MODS):  Medical Non­ Deployable (MND), Dental Classification Module (DENCLASS}, Health Readiness Record (HRR), Medical Data Protection System (MEDPROS), Soldier Patient Locator (SPL), and the Warrior Transition Report (WTR). The Case manager shall be familiar with the regulation and policies that apply to Individual Medical Readiness (IMR) and deployment.


·          Bachelor’s Degree

·          RN presently serving in National Guard reserves

·          Demonstrate ability to manage multiple tasks

·          Demonstrate experience in general planning, evaluation, analyzing, and implementing government programs

·          Active Secret clearance

·          Must have at least 3 years of leadership/supervisory experience
