PURPOSE OF POSITION: The Community Health Worker (CHW) is responsible for helping patients and their families to access and navigate the healthcare system. The CHW will work closely with primary care teams and other agencies to ensure the patient receives high-quality, comprehensive healthcare. This position will play a vital role in the clinics quality improvement projects.
ESSENTIAL JOB FUNCTIONS:
Responsible for establishing trusting relationships with patients and their families to effectively assist them with their healthcare concerns.
Participates in the intake process and interviews patients for a “needs assessment”.
Provides ongoing follow-up, and encouragement to help motivate patients to comply and actively participate in the developed plan of care.
Follows ups with health care team as needed to address concerns or to provide updates.
Consistently documents activities, services provided and other pertinent data in the patient’s medical record.
Helps patients in utilizing community resources related to their healthcare needs.
Assists patients in scheduling appointments, reminders and may on occasion accompany patients to appointments.
Helps facilitate communication between the patient and the healthcare team.
Is focused on quality outcomes and plays an active role in quality improvements projects.
Perform monthly reports not limited to:
Meaningful Use: work with Clinical Supervisor to monitor measures/adjust work flows as well as annual reporting.
Monitor EOCCO patients- incentive measure reporting
Moda messaging: monthly patient reports on EOCCO patients
Arcadia – Qlik report (Epic)
Follow’s Edie reports
Attend monthly LCAC meetings
Home visits when needed.
Meets with Symmetry Care Representative daily.
High risk children, who have not been removed from home, are tracked to ensure appointments are kept and child is followed.
Attend daily inpatient rounds.
Provides basic Medical Assistant duties when needed.
Works collaboratively and effectively with the health care team.
Performs other duties as assigned.
JOB QUALIFICATION REQUIREMENTS: Has a broad knowledge of principles and practices in the medical field. Must possess excellent time management, organizational, and work planning skills. Knowledge of community health resources in rural populations and cultural competency. Knowledge of health care regulations and guidelines, word processing, spreadsheet and database application software. Demonstrative experience in a healthcare.
SPECIAL REQUIREMENTS: At time of hire or within 12 months of hire must be able to successfully complete CHW certification program. Must have a valid Oregon driver’s license.
DESIRABLE REQUIREMENTS: Experience in healthcare or social work settings providing direct patient contact and/or service. Highly independent, organized worker. Ability to inspire confidence in others. Demonstrated communication and public relations experience.
