Summary:
Provides Care for patients referred by the primary care provider, then returns care of the patient to the primary care physician at the time of discharge. Consults in creating a care plan while patient is receiving hospital at homecare services. Responsible for managing the care of hospital at home patients and works collaboratively with nurse case managers, care coordinators, consulting and primary care physicians and other healthcare professionals who comprise the patient¿s care team. The hospital at home physician leads the care team in providing efficient and compassionate care.

 

Responsibilities:
*Oversees and is accountable for the overall quality of care and clinical outcomes of patients admitted to hospital at home program.

* Oversees and is accountable for all resource utilization within the hospital at home program.

* Ensures both patient and provider satisfaction and develops plans for improvement.

*Participates in the development and administration in the department¿s budget, capital allocation, clinical, and service quality performance.

*Admits patients into the hospital at home services or in the patient¿ home.

*Organizes, attends and facilitates care team meetings Attend and participate in all regular scheduled service meetings

*Provides real time feedback to PCPs regarding clinical and operational guidelines to optimize patient care and clinical efficiency.

*Maintains coding and encounter data standards for completeness, accuracy, timeliness and reimbursement level to include legible documentation

*Adheres and maintains all provider credentialing and QI standards.

*Collaborates with the Medical Director and/or other physicians to assure proper provider staffing.

*Mentors and educates all new hospital at home providers regarding service standards.

*Enhances professional growth and development through participation in educational programs, literature review, in-services, and workshops.
