Position Summary:
To assure the timely and accurate dispensing and administration of medications and IV solutions to patients pursuant to a therapeutically valid prescriber’s order and in accordance with all state and federal regulations as well as established peer standards, hospital policies, procedures and protocols. Working with the Pharmacy Director and Clinical Coordinator manages the daily operations of all areas of drug distribution, and clinical drug monitoring including Pharmacy Technicians and Pharmacy Externs/Interns. Also provides assistance to technicians generating and entering home medication lists per departmental guidelines; follows pharmacy policies, procedures and protocols; participates in Performance Improvement activities and maintains readiness for The Joint Commission (TJC) accreditation.
Key Reponsibilities:

-Clinically responsible for evaluating therapeutic rationale of all medication orders, providing information/education as needed, and for producing accurate medication administration records.
-Answers telephone calls in an appropriate, timely and respectful manner.• Accurately follows various Clinical Pharmacy Protocols; Vancomycin, Amino glycosides, Weight Based Heparin, anticoagulation, Parenteral Nutrition, Adverse Drug Reaction Report, Renal Dosing.
-Verifies all aspects of medication order & follows e-MAR policy
- Identifies and participates in Performance Improvement activities
-In response to new information and/or specific requests, educates medical, nursing, and pharmacy staff in a timely and accurate manner utilizing appropriate reference sources and/or medical literature..
-Is familiar with pertinent pharmacy, nursing service, and house wide and system policies and procedures to facilitate accurate medication administration and optimal therapeutic results.
- Distributive responsibility for accurate and timely administration of Medications, IV solutions.
- Monitors efficient receipt of physician’s orders, processing of orders and delivery of medications.
-Checks all medications for hourly distribution rounds.
-Knowledge of the Alaris Smart Program software and drug library.
-Knowledge of Parenteral nutrition and entering orders into the CAPS software system.
-Knowledge of Pyxis MedStation and Pharmacy console; demonstrates ability to load/unload medications, troubleshoot common problems (drawer failure, controlled substance discrepancy, etc), troubleshoot Pyxis/Meditech issues, reviews “meds ordered not loaded” reports for prompt attention
- Obtains “short” inventory items or provides therapeutic alternative, provides necessary documentation to assure product availability and appropriate management.
-Monitors inventory usage, rotation of stock and disposal of outdated, recalled or poorly labeled products.
-Works collaboratively with Pharmacy Technicians to insure items are written on list to be ordered from wholesalers.
-Supervisory/Advisory responsibility to coordinate activities and promote teamwork within the Pharmacy and other hospital departments for optimal patient care.
-This includes, but is not limited to supervision of Pharmacy Technicians performing home medication list generation and home medication list entry into the hospital information systems in accordance with departmental guidelines and competencies.
-Maintains safe and pleasant working condition.
-Completes mandatory house wide competencies.
-Assists in the continuous training of pharmacy staff through observation & mentoring.

-Provides information/documentation pertaining to performance issues for all professional staff including pharmacy, nursing, physician, and other professionals.
• Mentors new pharmacy staff, both pharmacists, technicians and students.
• Participates in Performance Improvement activities within the Pharmacy Department and interdisciplinary groups.
• Participates in assigned special departments projects and committee work (Medical Staff Committees and Hospital Committees/Task Forces).
• Completes periodic competencies: both house wide and specific clinical competencies.
• Maintains professional competence through continuing education as required by the Commonwealth of Massachusetts and the Pharmacy Department. .
• Recognizes safety hazards and takes corrective action whenever a hazard exists. Provides for personal safety by having a working knowledge of pharmacy & hospital wide safety, disaster and infection control plans.
• Serves as a role model and resource for other staff by sharing knowledge with colleagues.
• Seeks education and learning experiences in areas of identified growth needs and reviews progress during annual evaluation.
• Responsible for required documentation. Maintains records as required by State and Federal regulations and departmental policies including: controlled substances, dispensing records and billing records as determined by periodic review.
• Appropriateness of controlled substance of C-2 Safe
• Completes compare report each shift and resolves any discrepancies prior to completion of shift, completes weekly county of C-2 Safe and monitors accuracy and documentation of controlled substances for all other areas of hospital
• Receives controlled substances from wholesalers and enters into perpetual inventory. Completes and files DEA #222 form when controlled substances are received.
• Is familiar with policy & procedures for reporting loss of controlled substances, including completion of DEA form #106; uses and documents use of refractometer readings for all controlled substances returned to the Pharmacy Department.
• Enters receipt and dispensing amounts of Tax Free Alcohol from running inventory, verifying account
• Documents all Loan/Borrow transactions from other facilities in Loan/Borrow Book. Messages appropriate person concerning transaction and takes responsibility to return borrowed items at earliest opportunity
• Maintains combination control. Maintains restrictions regarding admittance to pharmacy and observes non-pharmacy personnel allowed admittance
• Consistently maintains work schedule and attendance requirements as stated in the Medical Center’s policies.
• Observes proper time limit for lunches and breaks. Reports to work on time and/or is ready to report at change of Shift as documented by time card and supervisor’s observance

Required Knowledge & Skills:
Hospital Pharmacy experience including Unit Dose, compounding of Sterile products, per the most current USP regulations and computer technology as it relates to pharmacy practice preferred. Excellent interpersonal and Communication skills (written & verbal) required.
Bachelor’s Degree in Science/Pharmacy or Doctor of Pharmacy (Pharm D) from an Accredited School of Pharmacy
Hospital experience including Unit Dose, compouding of sterile products, per most current USP regulations, and computer technology as it relates to pharmacy practice.
